To reduce the waiting time for your office visit, we encourage you to print and complete the registration forms sign and bring them with you along with your insurance card on the day of your appointment. If you have access to a fax machine, you may also fax them to us at (713) 797-9498.

If you cannot print the registration forms from your computer please arrive for your appointment approximately 15 minutes prior to your appointment time in order that you may complete your paperwork here in our office.
